Important! Intranasal Flu Vaccination Consent Form
Dear Parent/Guardian,
Please see the attached electronic link from the School Nursing Service for consenting to this year’s intranasal flu vaccination for your child.
The School Nursing team will be visiting Bedwas on 12/12/2022 and Rhydri on 13/12/2022, to give children (Reception to Year 6 age) this vaccination.
Parents are required to complete the e-form below to opt in or out of your child receiving this vaccination.
